The Office of the Seniors Advocate maintains an online Long-Term Care Directory that lists information about publicly subsidized long-term care homes in British Columbia. It contains information from a variety of sources, including the facilities themselves. The information is updated every year in the fall.
This year we are expanding the directory to include registered publicly subsidized assisted living and are asking you to provide information about your residence by completing this questionnaire. The questions are about general demographics, resident and facility councils, accreditation, complaints and concerns, food services, facility units, resident personal fees, languages spoken by staff, parking availability, and contracted services. There is also an opportunity at the end of the questionnaire to add any comments that will help us better understand the context of the information provided. This questionnaire only needs to be completed this one time, but you will be given the opportunity to review and update any information prior to the public release each year.
If you cannot complete the entire questionnaire at once, you can save and resume it at a later time by clicking on "Resume later" in the top right corner of the screen.
This information request must be completed pursuant to Sections 3 and 7 of the Seniors Advocate Act and submitted by end of day on May 20, 2022.
If you have difficulty accessing this questionnaire or believe you will not be able to meet the deadline for completion, please contact the Office of the Seniors Advocate directly: OSA.email@example.com or 778-974-4203
